What this site collects

Analytics events fire through Google Analytics 4 only once the practice activates the measurement tag; until then the tag is an inactive placeholder and no analytics data is collected. The hosting server keeps standard server logs, such as requested URLs and IP addresses, as virtually all web servers do.

What the referral form transmits

The referral form sends its submitted fields by email to the practice. Nothing you submit is stored in a database on this site, and the site never writes the content of a submission to any log. The form uses a short-lived session cookie for its security checks; that cookie holds no personal information, only a random token and rate-limit timestamps.

Do not submit clinical records

The form is for logistics only: names, callback numbers, a body region, urgency, and brief scheduling notes. Do not submit clinical records, imaging reports, diagnoses, or identifying details beyond a first name and callback number. A plain web form is the wrong container for that information; the practice coordinates records exchange after contact through channels agreed with your office. The form actively rejects notes that read like clinical detail.

Cookies

Cookies on this site are limited to the analytics cookie described above, which exists only after the practice activates the tag, and the referral form's session cookie described above. There is no advertising, no tracking pixel network, and no account system.
