There are three ways to send a referral: the short form below, a call, or a text to (305) 707-8484. Whichever channel you use, the practice contacts the patient directly to schedule, and a consult note returns to you after the visit. The form asks for logistics only, so it takes about two minutes; what belongs here in the first place is spelled out on the capabilities page.

How the referral moves

  1. Send the referral. Use the form below, or call or text (305) 707-8484 if that is faster for your office. Out-of-area and visiting-patient cases follow the same path; care coordination explains how the loop closes when care finishes back home.
  2. The practice contacts the patient. Scheduling happens directly with the patient. Same-day or next-day appointments may be available. Availability changes, please call ahead to confirm.
  3. You receive a consult note. After the visit, a consult note goes back to you with findings and the plan. Records exchange is coordinated by phone or the practice's standard secure channels once contact is made.

The form

The form sends a referral notice, nothing more. After you submit, the practice contacts the patient and confirms details with your office if anything is unclear. Do not attach or paste clinical records anywhere in it: records are exchanged after contact, through channels agreed with your office.
